Undergraduate Forms Note: All Undergraduate petitions are submitted to the Student Services Secretary in AB 112. Educational Policies Committee Petition This form is used to petition for exceptions to policy (E.g., Request for credit overload; Request for transfer of credit, etc.) Students must submit a completed, signed original with three (3) copies stapled to the back of the petition or review will be delayed until copies are provided. Petition to Transfer Credit from Another Institution Students must submit a completed, signed original and two (2) copies stapled to the back of the petition or review will be delayed until copies are provided. Undergraduate Late Add/Drop Petition Students must submit a completed, signed original and one (1) copy stapled to the back of the petition or review will be delayed until the copy is provided.
